Clinical Supervisor-Home Health - Community Health Systems (Crutchfield, KY)

Title: Clinical Supervisor-Home Health Location: KY-Fulton (Paducah region) The Clinical Supervisor is responsible for certain managerial functions and all clinical aspects of client care. Coordinates all services and serves as a case manager. Maintains documentation related to recertification for all benefit coverage for clients. Determines eligibility of clients in meeting criteria for services. Serves on committees as assigned and supervisors staff competency as directed. Services are provided to all ages but primarily to the elderly population. Education: Associate''s Degree in Nursing Bachelor''s of Science in Nursing preferred Experience: Minimum of two (2) years of clinical experience. Minimum of one (1) year case management/ Supervisory experience preferred. Computer experience preferred. Licenses/Certificates: Licensed as a Registered Nurse in the state of practice, Valid drivers license, auto insurance, current CPR certification.
